Addressing Racial and Ethnic Health Disparities in Ohio

Health Disparities are the disproportionate incidence of disease, disability and death among a particular population or group when compared to the proportion of their population. Historically, this term has referred to racial and ethnic populations. This webinar will explore health disparities and how they can be addressed by primary care practices.

This webinar is part of a series of quarterly patient engagement webinars hosted by Health Services Advisory Group in partnership with the Patient Engagement Learning Center of the Ohio Patient-Centered Primary Care Collaborative (OPCPCC).  The webinar will begin with a half-hour presentation at 12:15 p.m. and will conclude with a 15-minute Q&A session. Angela C. Dawson, MS, MRC, LPCC, Executive Director of the Ohio Commission on Minority Health, will serve as the presenter.  Participation is limited to the first 150 who register.  Questions may be directed to PCMH@odh.ohio.gov or 614.644.9756.

Objectives

By the end of this webinar, participants will be able to:

  • Define key terms such as health disparities, health inequities, social determinants of health, and health equity.
  • Recognize the importance of addressing health disparities within the patient-centered medical home (PCMH) model.
  • Identify strategies for addressing health disparities and moving toward health equity.
